Job Description

Physicians for home bound patients, is a fast growing practice of compasionate and caring Physicians who deliver quality care for homebound patients mostly in assisted living facilities and in homes of qualifying patients. We are looking for Physicians interested in type of services we offer. We assign you a zone of Assisted Living facilities and Homebound patients to care for. Duties include but not limited to Treatment of all chronic illnesses
Treatment prescriptions and refills
Arranging for diagnostics tests
Coordinating Hospital admissions
Immediate medical care upon discharge from hospital
Coordinating with Homecare agencies on skilled care (e.g. nursing care, social workers)
Providing prompt attention to Certification forms for Home health care agencies.
