The group is located on the Eastern Shore of Maryland which provides access to health care through the delivery of comprehensive medical, dental and behavioral health care services. The network has expanded over the past 26 years to include medical services delivered at seven primary care offices, seven School-Based Wellness Centers and through outreach services provided by the Migrant Health Program. The integration of behavioral health with primary care services is being piloted , with plans to expand behavioral health services to all medical sites in the future. The comprehensive Oral Health Program (OHP) provides full dental services and preventative oral health services through a School-Based Dental Program in public schools in all three counties. All offices and program sites are accredited by the Joint Commission for Accreditation of Healthcare Organizations (JCAHO).
