Job Description

Full time salaried position with a Geriatric Extended Care team (currently consisting of one physician/geriatrician and three ARNPs). Partnership track in 2 and competitive salary. Work week is 5 days per week. Currently no night call (future plans may lead to night call responsibility with corresponding adjustment to decrease work week requirement). Responsible for patients in post-hospital subacute care unit and 2 – 4 skilled nursing facilities. Flexible opportunity for inpatient and/or outpatient practice, either geriatric consultation or primary care, depending on individual interests of the applicant. Geriatrics CAQ desirable but not required. Weekly Extended Care team meeting (schedule coordination, planning, CQI, utilization management, in-services)
Opportunity for program development and innovation in conjunction with other senior health care initiatives. Participation on Senior Care Advisory Committee to promote clinic-wide senior health care initiatives.
