Palliative Care NP Needed in Medford, OR

Coverage Type: Scheduled Clinical Hours Plus Call
Schedule: Looking for an NP that can provide onsite coverage at the Providence Hospice location in Medford full time. They will be back up for Portland. We will need coverage Monday-Friday (8am-5pm with a one hour unpaid lunch that they can take when they see fit). They will participate in symptom management call on weekdays (5pm-8am), which is beeper. There is a potential that they will cover weekend call which would be from 8am-8am. Call schedule will be determined once confirmed. It may not be consecutive days in a row but the site is open to having a provider take call in a row. They will average 4 nights of weeknight call and 1 weekend a month.

Work Details:
· Support staff: Yes
· Providers Requested: 1
· EMR: Epic
· Skills required: Admissions and recertification face to face visits. They will be primary for symptom management for patients including medication prescribing. Partner with RN's for collaboration on patient needs

Practice Setting:
• Outpatient

Requirements:
· Board Certification: Yes
· License required: Active WA and OR License
· DEA: Ya – WA and OR
· Certifications required: BLS

Credentialing Timeframe: 30-90 Days
