• Clinical Description:
o Conduct pre scheduled in home health evaluations for Medicare Advantage, Medicaid, and select commercial health plans
o Visits include medication review, medical history review, and physical evaluation
o Providers do not prescribe medications, order labs, or alter existing treatment plans
o Pre populated, patient specific evaluation forms are provided
• Orientation Note:
o Orientation and training modules are unpaid

Schedule and Coverage
• Schedule: Flexible
o Monday–Sunday
o 7:00 AM – 7:00 PM
o No required nights or weekends
• Coverage Model:
o Short term or long term opportunities available
o Full time, part time, contract, PRN, temporary, or seasonal
• Patient Volume:
o 6–9 visits per day

• Board Certification: Required
• Licensure: Active Connecticut medical license required
