We are seeking a Primary Care NP for an ongoing assignment in Barlow, Oregon.

Start Date: July 2026
Contract Length: Month-to-month
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (40 hours/week with 1-hour lunch)
Practice Setting: Home Health / Telehealth
Assignment Details:
• Hybrid model: ~70% telehealth and 30% in-home visits
• Conduct annual risk assessments and chronic condition reviews
• See 12–14 patients/day via telehealth or 6–7 in-home (mix averages ~10/day)
• No prescribing or treatment—assessment-only role
• Adult and pediatric patient population
• Must specify county of interest at submission
• Epic/platform systems used for documentation
License: Active Oregon License Required
Requirements:
• NP required
• Must be comfortable with both pediatric and adult patients
• Must be enrolled/credentialed with Medicaid
• Strong assessment and telehealth experience preferred
