Job description

Start: September 28, 2026, or as soon as credentialed
Length: 13 weeks, with short-term and long-term arrangements available (full-time, part-time, contract, PRN, temporary, seasonal)
Coverage: Scheduled visits only - no call
Schedule: Fully flexible, Monday through Sunday within a 7:00-19:00 window; no nights or weekends required. Scheduling is coordinated directly between you and the health plan partner.
Volume: 6-9 visits per day
Practice Setting: Home health - evaluations conducted in the member's residence
Population: Adults

Houma area: Lafourche, Terrebonne
Lake Charles area: Allen, Beauregard, Calcasieu, Cameron, Jefferson Davis

Responsibilities:
- Conduct pre-scheduled in-home health evaluations in health plan members' homes
- Review medication lists and medical history
- Perform a physical evaluation
- Complete the pre-populated, member-specific assessment form for each visit
- No prescribing, lab ordering, or modification of existing treatment regimens

This model suits physicians looking to supplement income alongside other clinical or personal commitments, as well as those seeking a lower-intensity practice with full schedule control.

Requirements:
- Board Certified in a primary care specialty
- Active Louisiana license required and must be in hand - the client is not sponsoring licensure for this role
- New graduates accepted
- DEA not required
- Completion of onboarding modules prior to starting - approximately 10-12 hours, unpaid

Credentialing:
- Credentialing timeframe: approximately 6-8 weeks
