Urgent Dates (Clean File Required)
The following dates require a clean file to qualify for temporary privileges:
7/20–7/24
8/3–8/7

Providers without a clean file may still be considered for:
10/12–10/16
10/19–10/23
11/2–11/6
11/9–11/13
11/16–11/20
11/23–11/27
11/30–12/4
12/7–12/11
12/14–12/18
12/21–12/24
12/28–12/31
Schedule
Monday–Friday
7:00 AM–4:00 PM
No call
8 guaranteed hours per day
Overtime after 9 hours per day
Practice Details
Lower-acuity surgical cases
Requirements
Active state license
Board Certified or Board Eligible within 3–5 years of residency
For temporary privileges, providers with a clean file are required for the urgent July/August dates. Clean file means no significant malpractice settlements, NPDB issues, Medical Board actions, restrictions, terminations, DUIs, or drug-related issues.
Credentialing
Providers should be prepared to submit:
Current CV
24 months of relevant work history
Completed application
References available to respond promptly
All required privileging documents upon confirmation
