Job description

• Assignment: Ongoing, start ASAP
• Shift Type: 12-hour shifts (day or night)
• Schedule: Open to 1 week on / 1 week off
• Call: None
• Overtime: Not expected
• Cases: Bread & Butter, Endo, General, Ortho, Spine (optional), Pediatrics (optional), No OB
• Cardiac Capability: Required
• Supervision: 1:3 or 1:4
• Number of Anesthesia Rooms: 25
• Practice Model: Physician-directed
• Charting System: EPIC
• Credentialing: 45 days
• Medicare/Medicaid Enrollment: Required
• Cross-Credentialing: Must agree to 1 additional site
