Job Description:

A Speech Language Pathologist (SLP) will evaluate and treat residents/patients with temporary or long-term speech, hearing, swallowing, and cognitive disabilities in compliance with the client guidelines. They will evaluate the resident’s/patient’s speech, language, cognitive-communication, voice, swallowing, and fluency and develop treatment plans. They will treat the residents/patients based on a physician’s treatment plan.

Essential Functions:
Screens and evaluates patients/residents and develops appropriate care plan.
Under a physician's direction, provides direct patient/resident treatment based on established plan of care.
Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and/or swallowing or cognitive problems.
Administers, scores, and interprets specialized hearing and speech tests.
Instructs residents/patients, families, and caregivers to monitor speech and provide ways to practice new skills.
Completes required forms and clinical records in accordance with company policy and the client regulations.
Documents in patient records in an accurate and timely manner.
Devises special tools and treatments for the specific needs of patients.
Provides comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting.
Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.
Maintains resident/patient privacy pursuant to HIPAA.
Ensures punctuality and regular attendance for assigned shifts.

Other Functions:
Develops treatment plans, including short and long-term goals and methods to achieve identified goals.
Communicates the resident’s/patient’s speech therapy program to the family and/or guardian.
Keeps written documentation including evaluations, daily notes and weekly summaries.
Performs other duties as assigned.

Knowledge/Skills/Abilities
Current knowledge of effective speech therapy techniques.
Familiarity with clinical needs of geriatric population.
Strong clinical capabilities.

Requirements:
Bachelor of Science degree in Speech Language Pathology required.
Active license in Speech Language Pathology in the St of AZ.
Entry level candidates who are new to the field are encouraged to apply.

Preferred Skills:
2nd Step TB Test or QuantiFERON Gold Blood Test

Onsite
Monday- Friday 8:00am-5:00pm, no weekends
No Over Time Work
