Licensed Master Social Worker – Nursing Home Surveyor/Complaint Investigator – New Rochelle, NY 10801 (#25198)
Employment Type: Temp-to-Perm
Hourly Rate: $39.00/hr

✅ Qualifications
✔️ NYS Licensed Master Social Worker (required)
✔️ At least 2 years of clinical experience in a medical facility (nursing home preferred)
✔️ Experience in nursing home surveillance activities (2 years preferred)
✔️ SMQT Certification preferred—or ability to obtain within 12 months

✅ Key Responsibilities
✔️ Conduct surveys and complaint investigations (including abuse/neglect cases)
✔️ Evaluate compliance with federal and state long-term care regulations
✔️ Draft and submit Statements of Deficiencies (SODs)
✔️ Assess facility responses to Plans of Correction
✔️ Input findings into federal/state systems
✔️ Monitor Immediate Jeopardy situations and attend required meetings
✔️ Testify in hearings as needed
✔️ Adhere to DOH and IPRO guidelines
✔️ Travel to assigned nursing homes within the work schedule
