Board Certified NP/PA - REQUIRED

Provider must be local - REQUIRED, no exceptions 
Must have clean background for quick credentialing - REQUIRED no exceptions

Please provide availability based of dates listed above- REQUIRED
DEA - Verbal Confirmation Required at Name Clear 

MI License - Required at time of clear, no exceptions - not accepting IMLC
ACLS/PALS - REQUIRED
