Job description

• EMR: EPIC
• Physician Coverage: 7a-7p & 7p-7a daily
• NPPA Coverage: Sunday & Monday: 9a-7p & 2p-12a. Tues-Fri: 10a-10p
• 90 days to credential
• If immunization record is not available, a positive titer for each is required.
• Hepatitis B (Series of 3 doses)
• Measles/Mumps/Rubella (Series of 2 doses)
• Varicella/Chickenpox (Series of 2 doses)
• Tdap (TETANUS, DIPHTHERIA ACELLULAR PERTUSSIS) within last 10 years.
• Flu Vaccine for 2024-2025 season
• Or let me know if you need a declination form. (Religious or Medical reasons)
• TB completed within the last year.
• CSM will accept - (QuantiFERON, QuantiFERON-TB GOLD of T-Spot) or a 2-step skin test.
• Chest X-Ray if positive and a questionnaire that CSM will provide.
