2 APP needs for Family Medicine/Urgent Care in Modesto, CA

$100 - 110/hr + Travel Expenses

Facility: FQHC

Credentialing: 2-3 weeks

Start: 11/24 or 12/1

LOA: 3 month contract with option to extend

Work Schedule: M-F 8 AM - 5 PM with a 1 hour lunch
May be able to accommodate 4x10
** Admin time is built into this schedule **

Patient Volume: Approximately 24 patients/day; 29 are scheduled but there is a 20% no show rate. Walk-ins and same day appointments are also welcome if there is a huge no show rate, so may be up to 29 patients/day.

Procedures: FM providers should be comfortable doing Pap smears
Handle diabetes management, medication management, hypertension, and lots of chronic care cases
Be okay with the “alphabet soup” — basically covering for other providers’ patients and helping with their medication management when they’re out
Must be comfortable seeing all ages — from newborns to seniors (0–100!)

EMR: EPIC

License: CA

Certifications: BC, CA Furnishing License, Active CA DEA

Relevant Experience: Must have 1 year of EPIC Experience

Additional Information: Try to schedule the FPs with their own MA's. There will be 1 ½ MA's scheduled with them. The ½ MA is a float.

There are additional sites/locations for this need and the locum will have to be comfortable to switching locations, if absolutely needed. They will stay in the Modesto area.

First week onsite is dedicated to provider onboarding, epic training. There is also a 3 week patient ramp-up with the providers patient panel.

75% of the population is Spanish speaking and Fillipino. They have interpreter lines and virtual translation as well. Most of their permanent staff is Spanish speaking or able to understand Spanish.

4 hours/ week are dedicated to administrative time.
