FNP Needed in Cedar Rapids, IA (70% telehealth)

Job description

Iowa (IA):
Counties - Linn, Polk, Black Hawk and Scott.

Contract Lengths: month-to-month with the option to extend

Position Highlights:
Full-time (40 hours/week) Monday–Friday
8 am-5 pm with a 1-hour lunch
One-hour lunch break included
Must be comfortable seeing pediatrics and adults
70% Telehealth 30% in Home (Home Health)...If providers live in the county, scheduling will try and schedule in home visits, but telehealth will be a main component
Schedules are released 2–3 weeks in advance
Open to full-time or part-time providers
Paid for a full day regardless of patient volume (providers must enter availability into Salesforce to be paid for the full day)
1–3 hours of paid orientation, all additional ongoing education modules are expected to be completed during downtime within working hours
Must be enrolled/Credentialed in Medicard (please highlight in submission and confirm with provider)

Role Overview:
Conduct 5–6 in-home health assessments per day
Perform annual risk assessments and chronic condition reviews
Assessment-only role — no treatment or prescribing required
Escalate urgent findings to the patient’s PCP
A1C testing only (if applicable; no additional screenings required)

Travel.
Mileage reimbursed at the IRS standard rate
Must have an active IA Medical License
