Job description

Tele/Home Health FNP needed in Kentucky:
Start Date: May 2026 or As soon as credentialed
Contract Length: Flexible
Schedule: 5x8hr days
Job Details: 70% telehealth, 30% Home health
Perform annual risk assessments and chronic condition reviews
Assessment-only role — no treatment or prescribing required
Escalate urgent findings to the patient’s PCP
Adults & Peds
Must be enrolled/Credentialed in Medicare

Ketucky License Required (We can help license you!)
Credentialing Timeframe: 60 - 90 days
Medical malpractice insurance, travel, and lodging provided
