Job description

📍 Location: KY
📅 Start Date: May 18th, 2026 (or as soon as credentialed)
📆 Contract: Month-to-month with option to extend
🕒 Schedule: Full-time, Monday–Friday (8a–5p with 1-hour lunch)
👥 Patient Population: Pediatrics & Adults (must be comfortable with both)
🏥 Practice Setting: Telehealth (70%) + Home Health (30%)
🧪 Procedures: Virtual visits, in-home evaluations, routine care
📞 Call: No call required
✅ Additional Information:

Home visits within a 30-mile radius of assigned location
Schedules released 2–3 weeks in advance
Paid for full day regardless of patient volume (must enter availability in Salesforce)
1–3 hours of paid orientation
Ongoing education modules completed during downtime
Must be enrolled/credentialed in Medicaid
