Job description

Tele/Home Health FNP needed in Kentucky:
Start Date: As soon as credentialed
Contract Length: Flexible
Schedule: FT (40hrs/week) M - F, 8a-5p
Job Details: Perform annual risk assessments and chronic condition reviews
- Assessment-only role — no treatment or prescribing required
- Escalate urgent findings to the patient’s PCP
- 70% telehealth, 30% Home health
- MUST BE COMFORTABLE SEEING PEDIATRICS AND ADULTS

Kentcuky License Required (We can help license you!)
Credentialing Timeframe: 60 - 90 days
Medical malpractice insurance, travel, and lodging provided
