📋 Contract Details
🔹 Anticipated Start Date: August 10, 2026
🔹 Contract Length: 26 weeks (flexible)
🔹 Schedule: Monday–Friday, 8:00 AM–4:30 PM
🔹 On-call coverage available and expected as needed to support 24/7 facility coverage
🔹 Employment Type: 1099 Locum Tenens
🔹 Client-provided travel is not included, local/drive-in candidates preferred
🔹 Submission deadline: July 23, 2026

🏥 Site Details & Clinical Scope
🔹 Skilled nursing and long-term care facility
🔹 Dedicated memory care unit
🔹 Serve Veterans, Veteran spouses, and Gold Star Parents
🔹 Perform routine and as-needed resident evaluations
🔹 Coordinate interdisciplinary care and participate in care planning
🔹 Complete documentation in the electronic medical record
🔹 Collaborate with facility administration, nursing leadership, and the interdisciplinary care team
🔹 Support continuity of care through onsite and on-call coverage

📋 Candidate Requirements
🔹 Active New York Nurse Practitioner license required
🔹 Family Practice experience preferred
🔹 Ability to provide onsite weekday coverage and participate in on-call rotation
🔹 COVID-19 vaccination required (medical exemptions only)
