Job description

📍 Location: WI
📅 Start Date: ASAP
📆 Contract Length: Month-to-month with option to extend for local candidates
🕒 Schedule: Full-time (40 hrs/week) Monday–Friday or part-time (20 hrs/week), 8a–5p with 1-hour lunch
Patient Population: Adults
🏥 Practice Setting: Telehealth (primary) with in-home visits
🧪 Procedures: Annual risk assessments, chronic condition reviews, A1C testing (if applicable)

✅ Additional information:
~70% telehealth / 30% in-home visits
Assessment-only role — no prescribing or treatment required
Conduct ~5–6 in-home health assessments daily
Escalate urgent findings to PCP
Schedules released 2–3 weeks in advance
Mileage reimbursed at IRS standard rate
Must be enrolled/credentialed in Medicare (please highlight on CV/submission)
Active Wisconsin medical license required
