Adell, WI – NP Telehealth/Home Health Assignment

Job description

Assignment Details:

Start Date: April 6th or ASAP upon credentialing
Duration: 4 weeks to start; month-to-month with option to extend
Schedule: Full-time, Monday–Friday (40 hours/week)
Hours: 8:00 AM – 5:00 PM (1-hour lunch included)

Position Highlights:

70% telehealth / 30% home health
Home visits within a 30-mile radius (Milwaukee area)
Equipment provided and shipped by client
Must be comfortable seeing both pediatric and adult patients
Schedules released 2–3 weeks in advance
Guaranteed full-day pay (must enter availability in Salesforce)
Paid orientation (1–3 hours); additional training completed during downtime
Must be enrolled/credentialed in Medicard (please confirm at submission)

Work Details:

Telehealth: 12–14 patients/day
Home visits: 6–7 patients/day
Mixed schedule: ~10 patients/day
Perform annual risk assessments and chronic condition reviews
Assessment-only role (no prescribing or treatment)
Escalate urgent findings to patient’s PCP

Requirements:

Active Wisconsin license required
