📍 Location: Milwaukee, WI (must reside within a 30-mile radius)
📅 Start Date: 4/6/2026
📆 Contract
🕒 Schedule: Monday – Friday, 8:00 AM – 5:00 PM (1-hour lunch)
Patient Population: Pediatrics and Adults
🏥 Practice Setting: 70% Telehealth, 30% Home Health
🧪 Procedures: Routine assessments and care management in both telehealth and in-home settings
✅ Additional information: Schedules are released 2–3 weeks in advance, allowing providers time to become familiar with assigned patients. Must be comfortable treating all ages.
