Advanced Practitioners - Primary Care
Job Details:
- Open to Nurse Practitioners and Physician Assistants in a 100% outpatient primary care setting focused on Adult Medicine (18+)
- Full-time position with weekdays only schedule - flexible scheduling options available
- No call required
- Build your own patient panel in a patient-centered environment with ample support for seamless integration
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
- Develop plans for further evaluation of health needs in consultation with physicians and care team members, including referrals to specialists
- Provide appropriate treatment for problems within scope of practice and refer to physicians when appropriate
- Work within a strong foundation of teamwork, respect, and commitment to patient care in a positive, open communication environment
- Utilize state-of-the-art EPIC clinical and informatics technology to promote improved patient outcomes and streamlined continuity of care
- Access to dedicated support through the Office of Advanced Practice
- Unrestricted State of Connecticut Medical License required (or eligible)
- APRN (Family, Adult/Geri Nurse Practitioner) or PA training required
- Minimum 1+ years of experience in a primary care or relevant outpatient setting required
