Quick Snapshot:
• Start Date: March 9, 2026
• Contract Length: 13 weeks; month-to-month extensions available
• Schedule: Monday - Friday, 8a-5p (1-hour lunch)
• Full-time 40 hours per week (part-time also considered)
• Practice Model: 70% Telehealth, 30% In-Home visits
• Counties: Linn, Polk, Black Hawk, and Scott
• Schedules released 2-3 weeks in advance
• Paid for a full day regardless of patient volume (must enter availability)

Role Overview:
• Conduct 5–6 in-home health assessments per day
• Perform annual risk assessments and chronic condition reviews
• Assessment-only role (no prescribing or treatment changes)
• Escalate urgent findings to patient’s PCP
• A1C testing only if applicable
• Patient Population: Pediatrics and adults

Requirements:
• Active Iowa license required
• Must be comfortable seeing both pediatric and adult patients
• Must be enrolled/credentialed in Medicaid
