Family Practice NP Tele/Home Health needed in Kentucky!

Job description

Start date: June 2nd, 2026, or as soon as credentialed

Contract length: Month-to-month with the option to extend

Position Highlights:
Full-time only (40 hours/week) Monday–Friday
8 am-5 pm with a 1-hour lunch
Equipment will be mailed out and provided by the client
70% telehealth, 30% Home health, will schedule home health visits within a 30-mile radius of the provided address
MUST BE COMFORTABLE SEEING PEDIATRICS AND ADULTS

Role Overview:
Perform annual risk assessments and chronic condition reviews
Assessment-only role — no treatment or prescribing required
Escalate urgent findings to the patient’s PCP

Must have an active KY Medical license
