Job description

Schedule / Call:
Full-time or part-time available
20–40 hours/week (flexible; client needs ~60 hours total coverage)
Mon, Wed, Fri 8a-5p.
Tues/Thursday 8a-8p.
Sat 8a-12p
May include 2 weekends per month

Assignment Details:
Outpatient primary care
25–30 patients/day
Adult + pediatric population (peds experience preferred)
Independent practice environment

Responsibilities include:
Managing patient panels
Preventive care and wellness visits
Acute and chronic care management
Referrals and care coordination
EMR: Epic

Requirements:
Family Nurse Practitioner
Ideally 3+ years experience
Comfortable working independently
Peds expertise preferred

Credentialing: 30–45 days
