FNP – Telehealth / Home Health | MO (Multiple Counties)

Job description

Schedule:
Start: May 18 or ASAP (post credentialing)
Month-to-month (option to extend)
Monday–Friday, 8:00am–5:00pm (1-hour lunch)
Full-time only (40 hours/week)

Assignment Details:
Coverage areas: Jefferson, Franklin, and St. Louis Counties
70% telehealth / 30% home health (within 30-mile radius)
Adult + pediatric patients (must be comfortable with both)
Telehealth: ~12–14 patients/day
Home visits: ~6–7/day (mixed avg ~10/day)
Assessment-only role (no prescribing/treatment)
Annual risk assessments + condition reviews
Escalate urgent findings to PCP
Equipment provided
Schedules released 2–3 weeks in advance
Paid full day regardless of volume (must log availability)

Requirements:
Family Nurse Practitioner
Active MO license
Must be credentialed/enrolled in Medicaid
Comfortable working independently
