Tele/Home Health Nurse Practitioner – Missouri
Start Date: May 18th or as soon as credentialed
Contract Length: Month-to-month with option to extend
Coverage Area: Jefferson County, Franklin County, and St. Louis County
Position Highlights:
Full-time only, Monday–Friday
Schedule: 8a–5p with 1-hour lunch
Equipment provided and mailed directly by client
Approximately 70% telehealth and 30% home health
Home visits scheduled within a 30-mile radius of provider’s address
Must be comfortable seeing both pediatric and adult patients
Schedules released 2–3 weeks in advance
Providers are paid for a full day regardless of patient volume
Paid orientation included
Ongoing education modules completed during downtime within working hours
Role Overview:
Assessment-only role with no prescribing or treatment required
Perform annual risk assessments and chronic condition reviews
Escalate urgent findings to patient’s PCP
Expected volume:
Telehealth only: approximately 12–14 patients/day
Home visits only: approximately 6–7 patients/day
Mixed schedule: approximately 10 patients/day
Requirements:
Active Missouri license required
Must be enrolled/credentialed with Medicare
Must be comfortable in both telehealth and home health settings
