💲 $135,000 – $150,000/year + $5,000 Sign-On Bonus

✅ Key Responsibilities
✔️ Patient Assessments – Conduct thorough exams and review medical histories
✔️ Care Planning – Develop and manage individualized treatment plans
✔️ Chronic & Acute Care – Monitor ongoing conditions and acute issues
✔️ Infection Control – Follow and support facility protocols
✔️ Patient & Family Education – Promote wellness and disease management
✔️ Team Collaboration – Work closely with physicians, nurses, and therapists
✔️ Documentation – Maintain accurate, timely clinical records

✅ Qualifications
✔️ Master’s or Doctoral degree (FNP or AGNP)
✔️ Active New York State NP license & board certification
✔️ New graduates welcome; geriatrics experience a plus
✔️ Strong EHR, clinical, and care coordination skills
✔️ Compassionate, detail-oriented, and culturally competent
