Assignment Details:
Location: Oxford, NY
Setting: 242-bed Veterans Home and Skilled Nursing Facility
Schedule: Monday–Friday, 8:00 AM to 4:30 PM
Coverage: Full-time onsite with shared 24/7 on-call coverage
Patient Population: Veterans, Veteran spouses, and Gold Star parents

Responsibilities:
Routine and as-needed resident evaluations
Care coordination and treatment planning
EMR documentation
Participate in interdisciplinary care planning
Collaborate with nursing leadership and facility administration
