Job description

Family Nurse Practitioner locum opportunity available in Barlow, OR with a hybrid telehealth and in-home care model.

Schedule:
• Monday–Friday, 8a–5p (1-hour lunch)
• Full-time (40 hrs) or part-time (20 hrs)
• No call
• Start: April 2026
• 3-month assignment with option to extend

Practice:
• Hybrid model: ~70% telehealth, 30% in-home visits
• 12–14 telehealth visits/day or 6–7 home visits/day
• Assessment-only role (no prescribing)
• Focus on annual risk assessments and chronic condition reviews
• Escalate care needs to PCP
• Flexible scheduling released 2–3 weeks in advance

Requirements:
• Board Certified Family NP
• Active OR license (licensing not available)
• Medicare credentialing required
• Comfortable with telehealth and home health workflows

Additional Info:
