Job description

Location: Multiple Counties Across Texas

Position: Nurse Practitioner or Physician Assistant

Start Date: As soon as credentialed
Assignment Type: Flexible – full-time, part-time, PRN, contract, travel, or seasonal

Schedule:
Flexible schedule, Monday through Sunday
No nights or weekends required
Shifts between 7:00 AM – 7:00 PM

Practice Setting:
Home Health

Position Overview:
Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
Responsibilities include reviewing medical history, medications, and performing a physical evaluation in the patient’s home
No prescribing, lab ordering, or changes to treatment plans required
Pre-populated, patient-specific forms provided for documentation

Work Details:
Average 6 to 9 patient visits per day
EMR: TBD

Travel:
Airfare, hotel, and rental car may be provided depending on eligibility and hours worked (FT ONLY)

Requirements:
Board Certified NP or PA
Active state license required
Master’s degree required
NPI required

Credentialing:
Timeline ranges from 7 days to 2 to 3 weeks
Name clear required
