Job description

Tele/Home Health Nurse Practitioner – Wisconsin
Start: ASAP (as soon as credentialed)
Length: 3 months with option to extend
Schedule: Monday–Friday, 8:00 AM–5:00 PM (40 hours/week, 1-hour lunch)
Setting: 70% telehealth / 30% home health (within ~30-mile radius, Milwaukee area)
Highlights:
Full-time only
Equipment provided
Schedules released 2–3 weeks in advance
Paid full day regardless of patient volume
1–3 hours of paid orientation (ongoing modules completed during downtime)
Role:
12–14 tele visits/day or 6–7 home visits/day (mix ~10/day)
Annual risk assessments & chronic condition reviews
Assessment-only (no prescribing)
Escalate findings to PCP
Must see both pediatric & adult patients
Requirements:
Board Certified FNP
Active state license
Must be Medicare credentialed
