Case Manager/ Discharge Planner
Job description
The Role
We are looking for a compassionate and organized Case Manager / Discharge Planner to join our growing care team. In this role, you will coordinate client care transitions, support continuity of care, and assist clients in connecting with community-based services and resources following treatment — all in a fully remote environment.
Responsibilities
Coordinate discharge planning and continuity of care for clients
Conduct 30-day follow-up care check-ins to confirm client connection to recommended services
Connect and collaborate with community resources and support programs
Serve as a liaison between clients, healthcare providers, and community partners
Assist clients in navigating available social and healthcare services
Maintain accurate, timely, and complete documentation of all client interactions and care coordination activities within the organization's designated systems
Communicate effectively with interdisciplinary teams to support positive client outcomes
Follow all crisis management and emergency response protocols, including mandated reporting obligations
Participate in required staff meetings, supervision (if applicable), and ongoing professional development
Comply with all clinical, administrative, and operational policies outlined in the Helen's Project Policy and Procedure Manual
Work Details
Employment Classification Available: Full-Time W-2: At-will employment
Remote Work: This is a fully remote position. Employees must maintain a professional, distraction-free, and HIPAA-compliant workspace at all times.
Scheduling: Flexible scheduling is available. Because many clients may work non-traditional hours, employees are expected to maintain scheduling flexibility to meet client needs. Each workday follows a 70/30 activity requirement — 70% client-facing and 30% administrative.
Pre-Employment Requirements All offers are contingent upon successful completion of a background check, credential verification, and onboarding orientation. A mandatory one-week onboarding training is required prior to beginning client-facing duties. No client-facing work may begin until full onboarding clearance is granted by Human Resources.
