Position Overview
• Specialty: Primary Care / Family Medicine
• APP Type: NP or PA (either accepted with primary care experience)
• Start Date:
o Preferred: July 1
o Can start as soon as credentialing is near completion
o Expedited credentialing requested
o Ideal candidate may start up to 1 week early to shadow clinic providers
• Contract Length: 4 months

Schedule & Hours
• Weekly Hours: 40 hours/week
o 36 hours direct patient care
o 4 hours administrative time
• Shift Length: 8 hour shifts
• Schedule Window: Monday–Friday, 7:30 AM – 5:30 PM
• Coverage Need: Full time for the duration of the contract

• Patient Volume: 14–16 patients per day
• Patient Population: Family Medicine – birth through end of life
• Practice Setting: Family Medicine Residency Clinic
o Includes FM OB and pediatric care
o Integrated behavioral health
o Collaborative Care Model
o Integrated clinical pharmacy and nursing support
• EMR: Epic
