FM physician coverage in Illinois

Specialty: FM Physician (from my understanding)-yes
# of providers- if we had multiple providers interested how many could the clinics take either PT or FT?- prefer one provider
BC or BE or both?- prefer BC but must be BE
Credentialing time frame- Depends on how fast I receive documents back. – We have a pre-app process where a 1 page document needs to be completed and returned with the list of documents (license, DEA, COI, etc). Once returned in entirety, it is reviewed by CEO, Chief of Staff & Credentials Chair. After their review the full app & supplemental forms are sent for completion. Once I receive everything back, I will begin verification process. The background check takes the longest to come back and will play a part in start date.
Schedule and clinic hours- 8a-6p – with 30 minute lunch so Sign up or sign in for contact information hrs per day
EMR- athena
Support staff-RN, MA, CNA, front desk
Patients per day- 25

• 3-6 month contract
• working at three locations close to each other
• 9.5 hour work days. 8-6, 30 min lunch break
