Family Medicine Provider Need in IL

Job description

Requirements:

· BC or BE in IM/FM

· BLS

· May be responsible for admissions

Overview:

· Skilled nursing facilities

· Schedule: Mon–Fri

o Minimum of 16 PPD; once reached, provider may leave (typically takes 6 hours)

· 3-month assignment with potential to extend

· Locations: One skilled nursing facility per day, rotating between Godfrey, Alton, Cahokia, Mount Vernon, and Mascoutah, IL

· 18+

· EMR: PCC and Arrista (new system)

· Credentialing: Approximately 2 weeks

· No T&L

• 2 week credentialing
• Long term or short term
• Per diem
