Job description

Start: December 9, 2026 (as soon as credentialed)
Length: Short-term or long-term; full-time, part-time, or PRN
Schedule: Flexible, Monday-Sunday, 7a-7p (no nights or weekends required)
Availability: Minimum of 60 hours per month
Volume: 6-9 visits per day
Practice Setting: Home Health

Duties:
- Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members
- Review medications and medical history and perform a physical evaluation in the member's home
- Complete a pre-populated, patient-specific form for each visit
- No prescribing or changes to current treatment; may be asked to collect labs

Open Areas:
- Carbondale area: Alexander, Gallatin, Hardin, Jackson, Johnson, Massac, Pope, Pulaski, Saline, Union, Williamson counties
- Quincy area: Adams, Brown, Hancock, McDonough, Pike, Schuyler counties
- Metro East (St. Louis area): Bond, Calhoun, Clinton, Greene, Jersey, Macoupin, Madison, Monroe, Montgomery, Perry, Randolph, St. Clair, Washington counties

Travel: Airfare, hotel, and rental car available depending on eligibility
Orientation: Orientation and training modules are unpaid

Requirements:
- BC or BE physician (MD/DO)
- Active Illinois license
- DEA not required
- Must have worked clinically within the past 2 years
- Must reside in the US
- Please indicate 3 counties of interest
- Credentialing: approximately 7 days to 2-3 weeks
