We are seeking a Permanent Family Medicine Physician in Plymouth, IN.

Rate: Permanent Placement Fee Rate Cap $25K
Shift: Days + On-call 1:7 by phone only

Position Details:
Patient Population: Pediatric through Geriatric
Setting: Outpatient Clinic
Responsibilities: Provide comprehensive outpatient care, perform exams, diagnostic tests, procedures, patient education, collaborate with physicians and staff, and participate in low-frequency on-call rotation.
Required Procedures: Basic procedures and lab services in clinic setting
EMR System: EPIC
Support Staff: RN, Medical Assistant, Receptionist
Days Per Month: 20–22 (typical)
On-Call/Weekends: 1:7 by phone only

Submission Requirements:
Board Certified or Board Eligible in Family Medicine or Internal Medicine (within 5 years of residency)
Indiana License (or willing to obtain)
BLS, ACLS, and DEA certifications (or willing to obtain)
Experience caring for patients of all ages
COVID vaccination
