Job description

Start: December 9, 2026 (as soon as credentialed)
Length: Short-term or long-term; full-time, part-time, or PRN
Schedule: Flexible, Monday-Sunday, 7a-7p (no nights or weekends required)
Availability: Minimum of 60 hours per month
Volume: 6-9 visits per day
Practice Setting: Home Health

Duties:
- Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members
- Review medications and medical history and perform a physical evaluation in the member's home
- Complete a pre-populated, patient-specific form for each visit
- No prescribing or changes to current treatment; may be asked to collect labs

Open Areas:
- Northern KY (Cincinnati area): Boone, Bracken, Campbell, Carroll, Gallatin, Grant, Harrison, Kenton, Nicholas, Owen, Pendleton, Robertson counties
- Western KY (Evansville area): Caldwell, Crittenden, Henderson, Hopkins, Union counties
- Northeast KY (Huntington area): Boyd, Carter, Elliott, Fleming, Greenup, Lawrence, Lewis, Mason, Rowan counties
- Lexington area: Anderson, Bath, Bourbon, Boyle, Clark, Estill, Fayette, Franklin, Garrard, Jessamine, Madison, Menifee, Mercer, Montgomery, Powell, Scott, Woodford counties
- Pikeville area: Breathitt, Floyd, Harlan, Johnson, Knott, Lee, Leslie, Letcher, Magoffin, Martin, Morgan, Owsley, Perry, Pike, Wolfe counties
- Somerset area: Casey, Lincoln, McCreary, Pulaski, Rockcastle, Wayne counties

Travel: Airfare, hotel, and rental car available depending on eligibility
Orientation: Orientation and training modules are unpaid

Requirements:
- BC or BE physician (MD/DO)
- Active Kentucky license
- DEA not required
- Must have worked clinically within the past 2 years
- Must reside in the US
- Please indicate 3 counties of interest
- Credentialing: approximately 7 days to 6-8 weeks
