This role centers on completing pre‑scheduled in‑home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members. Each visit includes reviewing medical and medication history and performing a focused physical assessment in the member’s home. With a 7a–7p scheduling window and no required nights or weekends, this position offers exceptional flexibility while allowing you to deliver meaningful, preventive care in a low‑acuity setting. Multiple contract structures are available, making it easy to tailor the role to your lifestyle and career goals.
