Truly BC/BE Primary Care - Full Time

8AM-5PM

AGES SEEN: Must be able to see Ages 2+ (will take FM/IM - but again required this age range and 3-5 Years Experience)

LOCATION: Oklahoma City, OK

EMR = E Clinical Works and would like the provider to have experience with this system already - big plus

Credentialing Items Required:
• State License
• DEA
• State Dispensing License (with their license number) - if they don't already have it they can obtain it prior to start date.
• BLS / ACLS
• COI
• Driver's License
• State Specific CSR
• Disclosure of Malpractice claims - if any
• National Background Check Results/Attestation
• NPDB Release Page 7-8
• NPI #
