Job description

Start: December 9, 2026 (as soon as credentialed)
Length: Short-term or long-term; full-time, part-time, or PRN
Schedule: Flexible, Monday-Sunday, 7a-7p (no nights or weekends required)
Availability: Minimum of 60 hours per month
Volume: 6-9 visits per day
Practice Setting: Home Health

Duties:
- Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members
- Review medications and medical history and perform a physical evaluation in the member's home
- Complete a pre-populated, patient-specific form for each visit
- No prescribing, lab orders, or changes to current treatment

Open Areas:
- Tulsa area: Adair, Cherokee, Craig, Creek, Delaware, Mayes, McIntosh, Muskogee, Nowata, Okfuskee, Okmulgee, Osage, Ottawa, Pawnee, Rogers, Tulsa, Wagoner, Washington counties
- Enid area: Alfalfa, Garfield, Grant, Kay, Major, Noble counties
- McAlester area: Atoka, Coal, Hughes, Latimer, Pittsburg, Pushmataha counties

Travel: Airfare, hotel, and rental car available depending on eligibility, plus $40 per day for gas
Orientation: Orientation and training modules are unpaid

Requirements:
- BC or BE physician (MD/DO)
- Active Oklahoma license
- Medicaid credentialed
- Must have worked clinically within the past 2 years
- Must reside in the US
- Please indicate 3 counties of interest
- Credentialing: approximately 7 days to 2-3 weeks
