Job description

This role involves conducting pre-scheduled, in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members. Visits include a comprehensive medication and medical history review along with a head-to-toe physical assessment, all documented using a pre-populated, patient-specific form.
