Provider Requirements
• Board Status: BC or BE in Family Medicine
• Licensure: Active Texas medical license required
• DEA: Active Texas DEA required
• Certifications: ACLS, PALS
• Background:
o No malpractice history of any kind accepted
• Interview Process:
o No formal interview
o CV reviewed by Chairman for decision
