Opportunity:
Physician - Family Medicine

Job Details:

• Board-Certified Family Medicine or Internal Medicine Physician sought to join a growing outpatient practice
• 100% outpatient role with no inpatient responsibilities
• Serve an underserved adult patient population in a hybrid primary care role
• Provide comprehensive primary care across multiple patient groups including Acute Response Care Clinic patients, post-discharge transitional care patients, and specialty clinic referrals
• Evaluate, diagnose, and treat acute and chronic medical conditions
• Manage patient care plans, preventive health screenings, and chronic disease management
• Coordinate care with specialists, care managers, and interdisciplinary healthcare teams
• Maintain timely and accurate medical documentation within Epic EMR
• Patient volume of approximately 20 patients per day
• Schedule: Monday through Friday, 8:00 AM to 5:00 PM
• EMR: Epic
• Minimal call coverage required — approximately one week of phone/beeper call annually
• Support quality improvement initiatives focused on patient outcomes and access to care
• Provide compassionate, culturally competent care to a diverse patient population
• Active Texas medical license required at time of submission
• Active DEA certification required
• Minimum of one (1) year of physician experience required
• Clean malpractice and license history highly preferred
