Physician - Family Medicine
Job Details:
- Board-Certified Family Medicine or Internal Medicine Physician sought to join a growing outpatient practice
- 100% outpatient role with no inpatient responsibilities
- Serve an underserved adult patient population in a hybrid primary care role
- Provide comprehensive primary care across multiple patient groups including Acute Response Care Clinic patients, post-discharge transitional care patients, and specialty clinic referrals
- Evaluate, diagnose, and treat acute and chronic medical conditions
- Manage patient care plans, preventive health screenings, and chronic disease management
- Coordinate care with specialists, care managers, and interdisciplinary healthcare teams
- Maintain timely and accurate medical documentation within Epic EMR
- Patient volume of approximately 20 patients per day
- Schedule: Monday through Friday, 8:00 AM to 5:00 PM
- EMR: Epic
- Minimal call coverage required — approximately one week of phone/beeper call annually
- Support quality improvement initiatives focused on patient outcomes and access to care
- Provide compassionate, culturally competent care to a diverse patient population
- Active Texas medical license required at time of submission
- Active DEA certification required
- Minimum of one (1) year of physician experience required
- Clean malpractice and license history highly preferred
- Visa sponsorship not accepted
