Physician - Family Medicine
Job Details:
- Permanent, full-time outpatient Family Medicine opportunity serving an underserved community population
- 100% outpatient with no inpatient responsibilities
- Monday through Friday schedule, 8:00 AM to 5:00 PM
- Approximately 20 patients per day across all age groups, including approximately 30% pediatric patients
- Clinical allocation of 1.5 days per week in Pediatrics and 3.5 days per week in Adult Medicine
- Provide comprehensive primary care services for pediatric, adolescent, adult, and geriatric patients
- Diagnose, treat, and manage acute and chronic medical conditions
- Conduct preventive health screenings, routine physical examinations, and wellness visits
- Provide pediatric primary care including well-child visits, immunizations, and treatment of common pediatric illnesses
- Develop individualized chronic disease management and treatment plans
- Educate patients and families on disease prevention, health maintenance, and treatment plans
- Coordinate care with specialists, ancillary services, and interdisciplinary healthcare teams
- Maintain accurate and timely documentation within Epic EMR
- Participate in limited annual phone/beeper call coverage, approximately one week annually
- Support quality improvement initiatives related to patient outcomes and access to care
- Deliver culturally sensitive and patient-centered care to a diverse community population
- Board Certification in Family Medicine required
- Active DEA certification and active state medical license required at time of submission
- Minimum of one year of physician experience required
- Clean malpractice and license history highly preferred
- At least one professional reference within the past 24 months required
