This outpatient community health center is associated with a nationally recognized Level I Teaching Hospital and is dedicated to delivering comprehensive, patient-centered primary care services to an underserved population. The facility fosters a collaborative, multidisciplinary environment with a strong commitment to preventive care, chronic disease management, and continuity of care within the community.

Opportunity:
Physician - Family Medicine

Job Details:

  • Permanent, full-time outpatient Family Medicine opportunity serving an underserved community population
  • 100% outpatient with no inpatient responsibilities
  • Monday through Friday schedule, 8:00 AM to 5:00 PM
  • Approximately 20 patients per day across all age groups, including approximately 30% pediatric patients
  • Clinical allocation of 1.5 days per week in Pediatrics and 3.5 days per week in Adult Medicine
  • Provide comprehensive primary care services for pediatric, adolescent, adult, and geriatric patients
  • Diagnose, treat, and manage acute and chronic medical conditions
  • Conduct preventive health screenings, routine physical examinations, and wellness visits
  • Provide pediatric primary care including well-child visits, immunizations, and treatment of common pediatric illnesses
  • Develop individualized chronic disease management and treatment plans
  • Educate patients and families on disease prevention, health maintenance, and treatment plans
  • Coordinate care with specialists, ancillary services, and interdisciplinary healthcare teams
  • Maintain accurate and timely documentation within Epic EMR
  • Participate in limited annual phone/beeper call coverage, approximately one week annually
  • Support quality improvement initiatives related to patient outcomes and access to care
  • Deliver culturally sensitive and patient-centered care to a diverse community population
  • Board Certification in Family Medicine required
  • Active DEA certification and active state medical license required at time of submission
  • Minimum of one year of physician experience required
  • Clean malpractice and license history highly preferred
  • At least one professional reference within the past 24 months required
