Family medicine provider needed at a CHC in Appleton, Wisconsin- coverage now through March

Primary Care
Board Certified Family Medicine or IM but must see ages 8+
WI license, CSR, DEA, and CPR/BLS
Must see at least ages 8+
ASAP start through about March 20th
Monday-Friday
Mon, Tues, Thurs: 7:30a-6:30p
Wed, Fri: 7:30a-5p
4 10’s is the preference
17-20 PPD
Epic EMR
Staff makeup: 2 PT MD’s (1 at a time), PT Ped, 3 NP’s, 4 RN’s, 1.5 MA’s/provider
Interview process: 20-30 minute phone screen, then 1 hr Zoom with staff

• Primary care duties- seeing ages 8+, 17-20 patients a day
• Work schedule is Monday-Friday
• Board certified in family medicine
