📋RESPONSIBILITIES
🔹 Provide direct patient care through in-office visits, telehealth, and occasional same-day appointments
🔹 Perform comprehensive assessments and physical examinations
🔹 Order and interpret laboratory testing and diagnostic imaging
🔹 Diagnose and manage acute and chronic conditions
🔹 Develop individualized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescription medications
🔹 Educate and empower patients regarding preventive health and functional medicine principles
🔹 Collaborate with health coaches, nutritionists, mental health providers, and other members of the clinical team
🔹 Build lasting patient relationships and contribute to a positive, team-oriented environment

🎓REQUIRED QUALIFICATIONS
✅ Active Oregon license as a Nurse Practitioner or eligibility to obtain licensure
✅ Board certification in Family Medicine, Adult-Gerontology Primary Care, or another applicable specialty
✅ Strong interest in preventive, functional, and lifestyle medicine
✅ Excellent communication and patient relationship skills
✅ Ability to thrive in a collaborative, fast-paced environment
✅ Proficiency with electronic health records
