Home Health NP/PA Needed in Illinois:
Start Date: As soon as credentialed
Contract Length: 3 months with option to extend
Schedule: Flexible, Monday – Sunday
40 hours a week
Day shifts only
Average Visits/Day: 6–9
Practice Setting: Home Health
Responsibilities:
Perform pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
Review medication history, medical history, and conduct physical evaluations
No prescribing, ordering labs, or altering current treatment plans
Pre-populated, patient-specific forms provided
